Eligibility:
If you have annual mammograms due to either a family history of breast cancer or you have had a previous diagnosis of breast cancer you will be eligible to take part in the study. We are currently recruiting from the following NHS Trusts

- North Bristol NHS Trust –Blood samples for the EMBED Study are taken at Southmead Breast Care Centre. If your mammogram is done at Tower Hill we will contact you to make alternative arrangements.

Taking part – what will I have to do?
Complete a consent form, a questionnaire and provide a blood sample. We expect to follow up most women for approximately 3 years. The number of years you are followed up will depend on when you were recruited and if you develop breast cancer. Those recruited later in the study will have less follow up.

If you would like to take part in the EMBED Study, the first thing you have to do is complete a consent form. You can do this by completing the form online following the link below, or contacting us to send a copy through the post. If you decide to complete the form on-line you will need the serial number found on the back page of the study leaflet.
The study leaflet was included with the paperwork for your mammogram appointment. If you no longer have this but would like to complete the form online please get in touch with the study team.
For Airedale patients, the first two letters of your unique code are capitals a and i as in the first two letters of Airedale.
We are experiencing some technical issues in that we are receiving blank consent forms.
If the system tells you you have reached the end of the form but you have not completed any details, you won’t have signed up for the study and we won’t know how to contact you. If this happens please do get in touch.
Blood samples
We would like you to provide a blood sample each year. Please note, the blood sample is for research purposes only: we will not be doing any diagnostic tests therefore there will be nothing to feedback at this stage. The blood sample will be taken either when you go for your mammogram appointment or by your GP. The leaflet you received tells you how your blood sample will be collected.
Please note that if your mammogram appointment is on the same day or within a few days of completing the consent form, we will try our best to accommodate you having your blood sample taken at the same time but it might be necessary to ask your GP for their help instead.
Questionnaire
We would also like you to complete a questionnaire. You can do this on-line or we will send you a copy of the questionnaire in the post. If you wish to complete the questionnaire online you will need the same serial number you used to complete the consent form. This questionnaire asks questions about your medical history. There will be a shorter questionnaire in the following years.
